We are looking for an hourly intern to support our Community Health Access Team (CHAT). The CHAT program seeks to provide personalized services to individuals who will not benefit long-term from reliance on the 911 emergency system. This may be achieved by connecting the individual to education and resources needed to prevent local Emergency Departments from becoming de-facto, primary care providers, services which hospital ERs are ill-equipped to provide practically or effectively. CHAT will identify potential patients, work with them to determine needs, and connect them to resources that may be better suited to provide for those needs. CHAT will combine social workers with emergency service personnel to care for both physical and mental health. In addition, CHAT will develop a patient-care plan focused on the whole person and connect them to appropriate services that may address their specific primary care issues.
